How to Pre-Register

After completing the registration process, you will have access to the information and services you need in a confidential and user-friendly platform.

• When your appointment is scheduled, you will receive a confirmation message via text or email.

• Three days before your appointment, you will receive a link to pre-register. To ensure an efficient and quick check-in, complete the entire registration process before your appointment.

• If your registration is incomplete, you will receive a reminder on the day of your appointment.

Check-In Humphreys Blvd & Southaven

  • 1. You can choose to complete your registration upon arrival, but to ensure a swift check-in process, we strongly recommend pre-registering before your visit.
  • 2. Upon your arrival for your appointment, keep an eye out for signs that say "Check-in." Look for the specific "Check-In" sign corresponding to your department, whether it's the clinic, physical therapy, or surgery center.
  • 3. Next, scan the QR code on the "Check-In" sign for your respective department. This will initiate a set of simple instructions to expedite your check-in process.
  • 4. If you're a clinic patient, you'll also receive a pager and be directed to the appropriate waiting room.

What to Bring

  • 1. Bring your current insurance card and photo identification, such as a driver’s license.
  • 3. Bring a referral if required by your insurance carrier.
  • 2. Bring a written list of medications with dosage you are currently taking, including vitamins and supplements.
  • 4. Bring any radiology films and CDs (X-rays, CT scans, and MRIs) you have that pertain to your current problem. If you arrive without these items, we may need to reschedule your appointment.
