The Frequently Asked Questions (FAQ) section answers some of the most commonly asked questions our staff and physicians receive. If you do not see an answer to a question you have, we encourage you to make a note so it can be discussed at length during your next appointment.

For urgent medical problems that occur after hours and cannot wait until the next business day, you may contact our office (901-522-7700), and an answering service will be available. An operator will contact the physician/provider on call. Any time you feel there is a life-threatening emergency, call 911. Do not delay by calling our office first.

Please bring your current insurance card, Driver’s License or Photo ID, any requested radiology films or CD’s (x- rays, CT scans and MRI’s) that pertain to your current illness.

A physician referral may be required for your appointment.

Some Semmes Murphey MD’s require Physician Referrals per their criteria. Semmes Murphey requires Physician Referrals for some insurance plans.

A Release of Information form can be downloaded from the Semmes Murphey Forms webpage or the same form is available at the Clinic.

If the patient is not a current Semmes Murphey patient, we cannot request medical records on behalf of the patient. The patient can pick up a Release of Information to complete and give to their Primary Care Provider (PCP).

No. Semmes Murphey does, however, specialize in Interventional Pain Management. Semmes Murphey doctors treat pain with Botox, PT, Trigger Point Injections, Epidural Steroid Injections, Facet Injections, Sacroiliac Joint Injections, and Sympathetic Ganglion Blocks.

Yes, we have an outpatient surgery center. It is located at our Memphis location. The address is 6325 Humphreys Blvd., Memphis, TN 38120.

Because of our patient volume, it could take about 24 hours for your nurse to follow back up with you.

Depending on the type of testing the patient had and/or the doctor’s preference, a nurse may call with results. Nurses respond within 24 hours.
